NDIS appointment request process

1. Submitting the above form with all details filled in (ensure you fill in all form fields, otherwise your submission may not be accepted).

2. Assessment of the form by our team (please allow 14 days for submissions to be assessed).

3. If your submission is accepted, you’ll receive our service agreement in your email inbox.

4. Please sign and return service agreement.

5. Our team will call to make your appointment.

If you have not heard from us after 14 days, please contact us on 0451 133 575, however we kindly ask you to please do not contact us until after 14 days has passed.

Our team will carefully review your submission to ensure it aligns with our NDIS service offerings. 

If your request is accepted, we’ll send you an NDIS service agreement for review and signature. 

Please be aware that processing times can take up to 14 days and our current wait time for an initial NDIS appointment is approximately 6 weeks. 

During your first visit, our expert podiatrists will assess your needs and discuss a personalised treatment plan tailored to your goals.
