Iliotibial Band Syndrome (ITBS)

Foot mechanics can impact the rest of the body, including contributing to Iliotibial Band Syndrome (ITBS). This condition occurs when the IT band, a thick tissue running from the hip to the shin,becomes irritated and inflamed.

The Iliotibial Band (ITB) plays a key role in knee stability, but repetitive movements like running can cause pain and inflammation on the outer knee.

Poor foot biomechanics can contribute to ITB syndrome by affecting leg alignment and movement patterns.

What causes Iliotibial Band Syndrome (ITBS)?

Iliotibial Band Syndrome (ITBS) is triggered by repetitive knee and hip movements, often from activities like running, cycling, hiking, and stair climbing. It occurs when the IT band becomes irritated due to strain at the knee attachment point.

Common contributing factors include:
  • Biomechanical issues: High arches, leg length discrepancy, and overpronation can misalign the leg, increasing stress on the IT band.
  • Muscle weaknesses: Weak hip abductors and glutes force the IT band to overcompensate, leading to strain.
  • Improper training: Sudden increases in distance, speed, or intensity, as well as excessive downhill running, overload the IT band.
  • Poor technique: Overstriding or limited hip rotation can place excessive pressure on the IT band.
  • Footwear & surfaces: Worn-out shoes, improper support, and running on hard or uneven surfaces can aggravate symptoms.

Symptoms of ITBS

The primary symptom of ITBS is pain on the outside of the knee, which worsens with activity.

Common symptoms include:

  • Pain on the outer knee during walking, running, squatting, or climbing stairs
  • Discomfort that flares up after exercise rather than during
  • Tenderness over the IT band where it crosses the knee
  • Pain radiating from the hip down the outer thigh to the knee and shin

As ITBS progresses, additional signs may appear:

  • Visible tightness in the IT band
  • Swelling on the outside of the knee
  • A grating sensation (crepitus) due to inflammation
  • Stiffness and difficulty straightening the knee
  • Muscle weakness or compensatory movement patterns

Addressing symptoms early is key to preventing long-term issues and further strain on surrounding muscles. Listen to your body’s warning signs.

Risk factors for ITBS

Several anatomical, biomechanical, muscle imbalance, and training factors can increase the risk of injury, including:

  • High foot arches (underpronation) or excessive foot rolling in (overpronation)
  • Leg length discrepancy causing uneven stress
  • Weak hip abductors, glutes, or core muscles
  • Tight IT band, quads, or hip flexors
  • Poor running technique or inefficient biomechanics
  • Frequent hill running or downhill exercises
  • Sudden increases in activity, distance, or intensity
  • Running or walking on hard, uneven surfaces
  • Worn-out or unsupportive footwear
  • Bowlegged alignment (increased Q-angle)
  • Femoral anteversion (inward-angled thigh bone)
  • External tibial torsion (outward-turned shin bone)

Caitlin graduated with a Doctor of Medicine degree after completing a Bachelor of Biomedical Sciences degree in Medical Science at the University of Western Australia.

Caitlin has spent most of her life dancing and recognises the importance of looking after one’s foot and overall health. She has a keen interest in foot health and biomechanics, enjoying her role as a permanent podiatrist at Elite Podiatry Mullaloo.

Having experience as a dance instructor, she looks forward to utilising her leadership skills integrated with her podiatric knowledge to build the foundation of providing high-quality patient-centred care.

Latifa completed her post graduate degree in Doctor of Podiatric Medicine at UWA in 2021. She enjoys treating calluses, ingrown toenails and musculoskeletal conditions with an interest in paediatrics. Latifa is very keen to share her knowledge with her patients to improve their quality of life.

Founder, Principal Podiatrist, Pharmacist & Lecturer

Marie, the owner and director of Elite Podiatry, is both a qualified Podiatrist and Pharmacist. She first established her founding Stirling clinic in December 2004 and has had exceptional success since. An inventor of a globally patented orthotic, Synxsole, Marie has been on Shark Tank and wrangled a deal with business guru Andrew Banks! She also has featured on Channel Seven’s Today Tonight, The Sunday Times, Smart Company, Lower Extremity Magazine in the USA and has been a regular on both ABC Radio and CurtinFM as their specialist Podiatrist.

Marie also lectures at the prestigious University of Western Australia in both the Pharmacy and Podiatry departments. She is also a mentor and regularly attends UWA Alumni events for graduating students.
